Patient Responsibilities
We welcome patient assistance
in helping us to help them, by informing us if they are unable
to attend an appointment, so that it can be offered to someone else.
Complaints
We offer a practice complaints procedure
to deal with complaints about the services we provide. This complies with
the NHS complaints procedure. Please contact the Practice Manager or
other member of staff if you have a complaint, which we will investigate
fully.
Confidentiality
We comply with the Data Protection Act
1998. Under this Act, we respect patient confidentiality. However,
should you wish all or part of your records to be disclosed to a third
party this can be done, but only with your signed consent.
Change of Personal Details
Please inform the surgery as soon as
possible if you change your name, address or telephone number. Any delay
can lead to confusion. |
How to Register at Eveswell Surgery
Registration Forms can be picked up from reception and an appointment
made for an evening surgery with a nurse for a New Patient Check. When
attending for a your check, please bring a urine sample with you.
Registration is complete after the GP signs the Registration Forms and
they are set off to the Health Authority.
Policy for Violent and Abusive Patients
At Eveswell Surgery we treat our patients with respect and we expect the
same in return although we do have a Surgery Policy regarding Violent
and Abusive Patients.
We will not tolerate any form of abuse whether it is verbal or physical
towards any member of the Practice Team. If this should happen we will
remove that patient from our list.
Discrimination
We do not discriminate on the grounds of
race, gender, social class, age, religion, sexual orientation or
appearance. Neither do we discriminate against patients with a
disability or medical condition. |
